How do I refer a patient as a General Practitioner?

You can refer patients via email to referrals@medconnecthmr.com.au or through HealthLink using our EDI: medconnecthmr. Include patient details and reason for referral.

Is there a cost for patients?

No, Home Medication Reviews are bulk-billed for eligible patients. There are no out-of-pocket expenses.

How long does a Home Medication Review take?

A typical HMR takes 60–90 minutes. The pharmacist will spend time reviewing all medications and discussing concerns with the patient.

How do I join as a pharmacist?
