MedConnect HMR operates as a referral coordination service connecting General Practitioners, patients, and accredited pharmacists for Home Medication Review services across Western Australia.

2. Services

MedConnect HMR facilitates Home Medication Reviews by:

  • Receiving referrals from General Practitioners
  • Coordinating with accredited pharmacists to conduct Home Medication Reviews
  • Managing the referral process and communication between healthcare professionals
  • Ensuring compliance with Medicare and professional standards

Our services are provided in accordance with the Australian Government Department of Health Home Medicines Review Program guidelines.

3. Eligibility and Referrals

For General Practitioners: Referrals must be made for patients who meet Medicare eligibility criteria for Home Medication Reviews. General Practitioners remain responsible for their patients' overall care and treatment decisions.

For Patients: Patients must be eligible under Medicare guidelines and have a valid referral from their General Practitioner. Patients consent to having their health information shared between their General Practitioner and the reviewing pharmacist.

For Pharmacists: Pharmacists must hold current AHPRA registration, Home Medication Review accreditation, and professional indemnity insurance to participate in our network.

4. Professional Responsibilities

All healthcare professionals using our services must:

  • Maintain current professional registration and accreditation
  • Comply with relevant professional codes of conduct and standards
  • Maintain appropriate professional indemnity insurance
  • Protect patient confidentiality and privacy in accordance with Australian privacy laws
  • Provide accurate and complete information in all referrals and reports
  • Act in the best interests of patients at all times

5. Privacy and Confidentiality

We are committed to protecting the privacy of all users. Personal and health information is handled in accordance with the Privacy Act 1988 (Cth) and Australian Privacy Principles.

6. Fees and Payments

Home Medication Reviews are bulk-billed services for eligible patients under Medicare. There are no out-of-pocket costs for patients.

MedConnect HMR receives a coordination fee for facilitating referrals between General Practitioners and pharmacists. This fee does not affect patient care or service quality.

7. Limitation of Liability

MedConnect HMR acts as a coordination service only. We do not provide medical advice, diagnosis, or treatment. All clinical decisions remain the responsibility of the treating healthcare professionals.

To the maximum extent permitted by law, MedConnect HMR is not liable for any loss or damage arising from the use of our services, except where such liability cannot be excluded by law.

8. Website Use

You agree to use our website only for lawful purposes and in a manner that does not infringe the rights of others or restrict their use of the website.

We reserve the right to modify, suspend, or discontinue any aspect of our website at any time without notice.
